Artemether saved a patient with severe falciparum malaria after quinine treatment failure (R III type of quinine resistance).

چکیده
Quinine has been one of the most effective drugs for the treatment of malaria since its introduction into clinical use several centuries ago. It is the drug of choice in severe falciparum malaria. Although the efficacy of the drug has gradually declined against malaria strains in various parts of Southeast Asia, resistance has not yet become a worldwide problem. Treatment failures from quinine in most cases, are low grade resistance (RI or R I1 type of response) which are mostly due to inadequate drug dosage (compliance reason). We report a successful treatment with artemether, in a patient with severe falciparum malaria after treatment failure from quinine (R 111 type of resistance).

Clinical Pharmacology of the Antimalarial Quinine in Children
Quinine is the best studied drug for treating severe malaria in very young children. Quinine may be administered in pregnancy and, at therapeutic doses, malformations have not been reported. Some strains of quinine from Southeast Asia and South America have become resistant. Quinine is the treatment of choice for the drug-resistant severe Plasmodium falciparum. The antimalarial mechanism of qui...

متن کاملPreliminary report: a comparative clinical trial of artemether and quinine in severe falciparum malaria.
Twenty-six patients with severe falciparum malaria were randomized to be treated with quinine or artemether. Twelve patients received quinine at the standard dose and fourteen patients received artemether intramuscularly at a total dose of 640 mg over 7 days. The patients were kept in the hospital for at least 7 days. Peripheral smear was performed 6-hourly until there was no parasitemia, then ...
متن کاملPharmacokinetic Properties of Artemether, Dihydroartemisinin, Lumefantrine, and Quinine in Pregnant Women with Uncomplicated Plasmodium falciparum Malaria in Uganda
Pregnancy alters the pharmacokinetic properties of many drugs used in the treatment of malaria, usually resulting in lower drug exposures. This increases the risks of treatment failure, adverse outcomes for the fetus, and the development of resistance. The pharmacokinetic properties of artemether and its principal metabolite dihydroartemisinin (n = 21), quinine (n = 21), and lumefantrine (n = 2...

We prospectively studied 803 Thai patients admitted to the Bangkok Hospital for Tropical Diseases to assess the safety, tolerability and effectiveness of treatments for strictly defined P. falciparum malaria. Patients were assigned to one of five treatment groups: (i) a 5-day course of intravenous artesunate in a total dose of 600 mg, Group Aiv; (ii) intravenous artesunate as in Group Aiv follo...
